Provide holistic primary and acute care across medical respite centers, clinics, mobile units, and outreach sites. Assess patients, order and interpret diagnostics, develop treatment plans, counsel on wellness, document care, coordinate transitions, and follow infection control and legal requirements while collaborating with physicians and interdisciplinary teams.

Provide clinical-team leadership for discharge planning, identify patient needs and community resources, connect homeless patients to housing and services, assist with benefits applications, document plans in medical records, and collect grant/project data.

Part-time internship supporting Compliance & Privacy operations at an FQHC. Tasks include assisting with audits, risk assessments, board presentation preparation, meeting minutes, compliance documentation, facility site surveys, and development of risk/quality reports while collaborating on process improvements and governance activities.

Provide direct primary care to homeless patients at a medical respite center, lead a multidisciplinary clinical team, coordinate and document care in the EHR, teach rotating students/residents, integrate behavioral health, oversee clinical policies, infection control, and quality metrics.

Provide outpatient primary care for individuals experiencing or at risk of homelessness. Perform assessments, generate diagnoses, develop treatment plans, document in EMR, coordinate multidisciplinary and community-based care, apply evidence-based practices, participate in QI, maintain licensure and certifications, and support care transitions and patient education.

Ensure organizational compliance by compiling and analyzing audit and incident data, identifying trends and control gaps, conducting internal audits and risk assessments, assisting with compliance training, monitoring regulatory and accreditation adherence, researching new laws, preparing compliance reports, and tracking remediation efforts to closure.

Provide clinical leadership and supervision for PTs, PTAs, and OTs across respite centers. Oversee recruitment, onboarding, performance, clinical operations, budget participation, quality improvement, compliance, and some direct patient care focused on people experiencing or at risk of homelessness.

Provide trauma-informed peer support in a medical respite program: assist admissions/discharges, support substance use recovery, co-lead groups, respond to urgent situations, document in the EHR, participate in supervision, and collaborate with clinical staff to help patients reach short- and long-term goals.

Provide clinical and clerical support at a medical respite center: communicate with nursing staff, transport patients to appointments, assist with personal hygiene and end-of-life one-to-one care, handle clerical tasks, inventory and stocking, dorm cleaning, patient laundry, meal coordination, phone coverage/triage, and other duties as assigned.

Provide patient support in a medical respite center: obtain and record vital signs, assist with admissions/discharges, document care in the EHR, liaise between providers and staff, track quality metrics and follow-up tasks, perform clerical tasks, and preserve patient confidentiality and dignity.

Provide clinical and clerical support at a medical respite center: assist with vitals and personal hygiene, transport patients, stock supplies, perform light clerical tasks and phone triage, support meals and laundry, provide one-to-one end-of-life care, and maintain communication with nursing/program management.

Provide primary and urgent care for people experiencing homelessness, performing assessments, forming differential diagnoses and treatment plans, coordinating multidisciplinary care, documenting in the EMR, addressing social determinants, and promoting wellness while maintaining licensure and certifications.

Manage denial resolution and revenue cycle improvement projects; train staff on billing processes and insurance eligibility; investigate and resolve complex payer-rejected claims; monitor and report denial percentages; prepare EHR reports; support timely claim submission and other revenue-cycle duties.

Provide compassionate, patient-centered primary care to individuals experiencing homelessness: assess, diagnose, and manage acute and chronic conditions; document in EMR; coordinate multidisciplinary care and community resources; apply evidence-based practices; maintain licensure, board certification, BLS, and participate in quality improvement.
