Directs daily operations of a medical respite center serving patients experiencing or at risk for homelessness. Leads nursing, administrative, transportation, dietary, and clinical support teams; oversees staffing, scheduling, budgets, supplies, admissions, discharges, regulatory compliance, quality improvement, reporting, and employee development. Provides clinical guidance to licensed nurses and ensures patient satisfaction, clinical quality, financial performance, and adherence to Arizona Department of Health Services standards.

Provides clinical direction and mentorship to nurses, ensures licensure and regulatory readiness, manages performance, and partners with leadership and Human Resources on operational and workforce goals.

Provides peer-based support to patients in a medical respite program, using harm reduction and trauma-informed approaches. Assists with admissions, discharges, recovery goals, behavioral health groups, urgent situations, documentation, supervision, and required training. Supports collaboration with nursing and behavioral health staff, maintains a safe therapeutic environment, and may provide transportation. Requires lived experience, peer support certification, behavioral health experience, BLS certification, and an Arizona driver’s license.

Provides psychiatric evaluations, diagnoses, treatment plans, psychotherapy, medication management, and addiction care for patients in clinics, respite centers, mobile units, street medicine teams, or partner organizations. Collaborates with multidisciplinary care teams, supervises advanced practice providers, documents care electronically, monitors patient progress, and ensures regulatory and ethical compliance.

Provide clinical and clerical support at a medical respite center: transport patients, assist with vital signs and personal hygiene, perform clerical tasks, stocking and cleaning, triage calls, support end-of-life care, coordinate meals, and communicate with nursing staff to ensure continuity of care.

Provide primary and urgent care for people experiencing homelessness, performing assessments, forming differential diagnoses and treatment plans, coordinating multidisciplinary care, documenting in the EMR, addressing social determinants, and promoting wellness while maintaining licensure and certifications.

Provide clinical and clerical support at a medical respite center: assist with vitals and personal hygiene, transport patients, stock supplies, perform light clerical tasks and phone triage, support meals and laundry, provide one-to-one end-of-life care, and maintain communication with nursing/program management.

Provide clinical-team leadership for discharge planning, identify patient needs and community resources, connect homeless patients to housing and services, assist with benefits applications, document plans in medical records, and collect grant/project data.

Provide direct primary care to homeless patients at a medical respite center, lead a multidisciplinary clinical team, coordinate and document care in the EHR, teach rotating students/residents, integrate behavioral health, oversee clinical policies, infection control, and quality metrics.

Part-time internship supporting Compliance & Privacy operations at an FQHC. Tasks include assisting with audits, risk assessments, board presentation preparation, meeting minutes, compliance documentation, facility site surveys, and development of risk/quality reports while collaborating on process improvements and governance activities.

Provide outpatient primary care for individuals experiencing or at risk of homelessness. Perform assessments, generate diagnoses, develop treatment plans, document in EMR, coordinate multidisciplinary and community-based care, apply evidence-based practices, participate in QI, maintain licensure and certifications, and support care transitions and patient education.

Provide clinical leadership and supervision for PTs, PTAs, and OTs across respite centers. Oversee recruitment, onboarding, performance, clinical operations, budget participation, quality improvement, compliance, and some direct patient care focused on people experiencing or at risk of homelessness.

Provide trauma-informed peer support in a medical respite program: assist admissions/discharges, support substance use recovery, co-lead groups, respond to urgent situations, document in the EHR, participate in supervision, and collaborate with clinical staff to help patients reach short- and long-term goals.

Provide patient support in a medical respite center: obtain and record vital signs, assist with admissions/discharges, document care in the EHR, liaise between providers and staff, track quality metrics and follow-up tasks, perform clerical tasks, and preserve patient confidentiality and dignity.
