Provide holistic primary and acute care across medical respite centers, clinics, mobile units, and outreach sites. Assess patients, order and interpret diagnostics, develop treatment plans, counsel on wellness, document care, coordinate transitions, and follow infection control and legal requirements while collaborating with physicians and interdisciplinary teams.

Provide clinical-team leadership for discharge planning, identify patient needs and community resources, connect homeless patients to housing and services, assist with benefits applications, document plans in medical records, and collect grant/project data.

Part-time internship supporting Compliance & Privacy operations at an FQHC. Tasks include assisting with audits, risk assessments, board presentation preparation, meeting minutes, compliance documentation, facility site surveys, and development of risk/quality reports while collaborating on process improvements and governance activities.
