Provides person-centered care to assisted living residents, including bathing, dressing, meals, laundry, daily routines, and medication assistance. Follows medication procedures, documents accurately, monitors resident needs, communicates changes, supports families, and collaborates with care team members. PRN staff cover open shifts caused by leave or call-offs while maintaining resident dignity, safety, confidentiality, and well-being.

Provides low-intensity progressive exercise and activity programs to older adult residents under PT or nurse direction. Monitors and adapts interventions, assists with transfers and mobility, documents activity, trains nursing assistants on restorative plans, communicates with therapy and nursing staff, and maintains resident safety, rapport, and HIPAA-compliant confidentiality.

Coordinates resident assessments and care plans across interdisciplinary teams in accordance with regulatory requirements. Oversees PPS, OBRA, RAP, and trigger assessments; transmits and maintains MDS records; facilitates care plan conferences; assesses prospective residents; supports quality assurance; updates policies and procedures; participates in government surveys; and performs nursing and administrative duties as needed.
