Provides low-intensity progressive exercise and activity programs for older adults under PT/nurse direction to maintain mobility and independence. Monitors responses, adapts interventions, assists with transfers and walking, documents activities, trains nursing assistants on restorative plans, and collaborates with therapy and nursing staff while following HIPAA and safety standards.

Plans and delivers meaningful activities and one-to-one programming for residents, coordinates small groups, manages supplies, drives for outings, supports the Director, documents resident responses, and performs clerical tasks.

Provide personal care and assistance with ADLs for assisted living residents, supervise and assist with medication administration under nurse supervision, monitor and document resident condition, report changes or concerns, maintain communication with supervisors and families, restock basic supplies, and participate in staff meetings.

Provide personal care and assist with ADLs for senior living residents; administer and document medications under nurse direction following MARs and the five rights; monitor and report changes in resident condition; maintain safety, confidentiality, and resident dignity; fill PRN shifts across days, evenings, nights, weekends, and holidays.

Lead and support clinical operations in a long-term care setting, acting for the DON as needed. Mentor and coach nursing staff, ensure regulatory compliance, participate in quality improvement, provide education, perform resident assessments, and maintain visible floor leadership to support resident-centered care.

Provide person-centered assisted living care: help residents with bathing, dressing, meals, laundry, mobility, and daily routines; assist with medication administration and documentation; monitor and report changes, support families, and collaborate with team to ensure resident safety and dignity.

Provide personal care and assistance with ADLs for assisted living residents, monitor safety and health status, supervise and administer medications per state certification and licensed nurse supervision, document care, restock supplies, report changes or concerns, and participate in staff meetings.

Lead clinical operations for a senior living long-term care neighborhood: support the Director of Nursing, mentor staff, ensure regulatory compliance and quality of care, provide education and training, participate in quality improvement, and serve as a visible, hands-on clinical leader for residents, families, and clinicians.
