Assess and coordinate transitions from hospital or facility to home health or hospice. Meet patients, educate on appropriate care settings, ensure clinical stabilization, perform medication reconciliation, document needs in HCHB, coordinate orders and community resources, support referral sources, and complete follow-up calls to reduce readmissions and improve patient outcomes.

Verify insurance coverage, obtain pre-authorizations, and coordinate with insurers, providers, and patients to secure timely approvals. Maintain records of authorizations, manage denials and appeals, and monitor status to prevent care delays and support billing and compliance.

Provide combined clinical and administrative nursing support: maintain patient records, coordinate appointments and admissions, assist with clinical tasks (vitals, medications), manage supplies, facilitate communication between patients and providers, and ensure regulatory compliance and infection control to support efficient facility operations.

Provide front-office administrative support for a home health office: answer calls, greet visitors, manage mail and supplies, schedule travel, perform data entry, assist leadership and marketing teams, coordinate meetings and events, and maintain records while ensuring confidentiality and excellent customer service.
