Charter Oak Health Center, Inc. is a federally qualified, Joint Commission-accredited nonprofit community health center serving residents of the greater Hartford area. Founded in 1978, it provides high-quality, patient-centered care regardless of ability to pay, including primary and urgent care, behavioral health, pediatrics, women’s health, dental, pharmacy, nutrition, eye care, case management, and services for underserved and homeless patients throughout the community.
