Provide community-based case management and psychosocial rehabilitation for adults with severe mental illness. Develop and implement treatment plans, monitor medications, provide crisis intervention, coordinate community resources and entitlements, assist with housing and daily living skills, document in EHR, meet billing and documentation quotas, and participate in team meetings and quality improvement activities.

Lead the finance team overseeing accurate financial reporting, billing, vendor payments, internal controls, audits, budgets, treasury functions, and compliance for a nonprofit. Manage relationships with external partners, implement best practices and technology, and supervise HR-related functions within Finance.

Provide crisis intervention and stabilization for adults in psychiatric crisis through individual and group therapy, assessment, triage, referral to community resources or higher care, documentation, liaison with outpatient providers, participate in on-call rotation, and assist with staff training and community education.

Provide full case management to help customers access program services: build relationships, monitor progress, assist with enrollment and benefits, connect clients to supportive services, maintain case notes, coordinate with staff, identify and resolve barriers, and promote workplace safety.

Create and maintain internal and external policy documents, work instructions, templates, and educational materials. Support documentation review/version control, conduct SME interviews, assist RFP and project research, perform data collection/analysis and ad hoc project tasks, and ensure high-quality, timely documentation under Quality Manager supervision.

Support IRIS participants to self-direct long-term care goals through home visits and phone contacts; complete ISSP, CIR, NOA and other documentation; coordinate providers and community supports; assist with Medicaid eligibility paperwork; attend trainings; provide coverage and training to other consultants; travel up to 50%.

Build therapeutic rapport, develop and monitor client-centered treatment plans, coordinate community resources, assist with housing and finances, provide crisis intervention, maintain documentation and billing, participate in team meetings, and transport clients as needed.

Manage daily operations of the Pediatric Skilled Nursing program including supervising clinical and non-licensed staff, staff development, scheduling and census management, intake and care planning, regulatory compliance (DHS 105, DCF 251), billing and prior authorization coordination, policy and procedure development, and partnership with community and transportation staff to ensure safe, high-quality pediatric care.
