Teams at Centers for Independence

Manage daily operations of the Food Share Employment and Training (FSET) program, supervise and train staff, ensure contractual compliance and quality service delivery, handle escalated participant issues, track referrals and resources, compile and QA program data, and maintain positive work culture.

Support IRIS participants to self-direct long-term care goals through home visits and phone contacts; complete ISSP, CIR, NOA and other documentation; coordinate providers and community supports; assist with Medicaid eligibility paperwork; attend trainings; provide coverage and training to other consultants; travel up to 50%.

Provide case management and psychosocial rehabilitation for adults with severe mental illness: develop and implement treatment plans, monitor symptoms and medications, provide crisis intervention and supportive counseling, coordinate community resources and entitlements, assist with housing and daily living skills, document care in EHR, meet billing and documentation quotas, and participate in team meetings and quality activities.
