Processes high-volume HR intake, payroll, and employment documents; corrects incomplete files; maintains applicant, worker, and client records; updates the FMS system; completes government registration applications; conducts background investigations; processes workers’ compensation applications; and provides customer service while meeting accuracy, productivity, and conversion standards.

Supports participants enrolled in the IRIS program through home visits, phone contacts, service coordination, care planning, documentation, incident reporting, and benefits eligibility assistance. Collaborates with participants, families, providers, community resources, and internal teams to achieve long-term care goals and ensure health and safety. The role requires regular regional travel, independent judgment, accurate recordkeeping, participant advocacy, and completion of required training and compliance activities.

Provides community-based case management and behavioral health support to individuals with severe mental illness and substance use disorders. Develops treatment plans, monitors symptoms and medication compliance, delivers counseling and crisis intervention, coordinates healthcare and community resources, supports housing and financial stability, maintains clinical documentation, and collaborates with treatment teams, families, and outside agencies. Requires client transportation in a personal vehicle and work across varied community settings.
