Provide consultation, intervention, and prevention services to Butler County school districts and residents; conduct assessments, develop treatment plans, coordinate referrals and follow-up, deliver classroom and community presentations, collect and enter required data, maintain confidentiality, and support outreach and marketing of county services. Position requires travel to schools and community sites and occasional evenings/weekends.

Provide blended case management to children, adolescents, and adults with serious mental illness: connect clients to services, coordinate and monitor care, complete documentation, participate in 24/7 on-call rotation, conduct community visits, and support crisis response to help clients remain in their homes and communities.

Provide person-centered supports coordination for individuals with intellectual/developmental disabilities: develop and maintain support plans, coordinate providers and community supports, monitor progress and incident response, assist with service and provider choice, and offer resources and family support. Position requires face-to-face meetings and county-wide travel.
