Provides blended case management to children, adolescents, and adults with serious mental illness or emotional disturbance. Connects clients and families with services, coordinates and monitors care, offers advocacy and crisis support, and helps individuals remain in home or community settings. Responsibilities include regulated documentation, quality compliance, time management, communication, community resource navigation, and participation in evening, weekend, holiday, and 24/7 on-call coverage.

Oversees crisis intervention services across Somerset and Bedford Counties, including compliance, risk management, quality improvement, staffing, supervision, training, performance evaluations, chart reviews, and clinical documentation. Coordinates program operations, coverage, policy adherence, community outreach, provider relationships, and service development. Provides direct crisis intervention support and participates in administrative on-call rotation, with occasional evening, weekend, and holiday flexibility required.
