Provide full-time ambulatory family medicine care in a team-based clinic (32-40 hours/week). Coordinate timely, appropriate outpatient care with other clinicians. No hospital work. Competitive salary and benefits; part-time options available.

Part-time, in-office role responsible for processing, reviewing, routing, and distributing incoming mail and correspondence. Researches patient records in the Practice Management system to identify recipients, coordinates distribution to internal departments and external locations, maintains HIPAA confidentiality, tracks documents, and supports mailroom organization and urgent communications handling.

Lead value-based care and quality improvement initiatives across practice sites. Coordinate QI programs, collect and analyze outcomes data, prepare NCQA and regulatory reports, support clinical teams and reporting tools, and implement evidence-based workflows to improve measures, reduce utilization, and optimize care team efficiency.
