Provide community-based DFS liaison and case management services for families affected by substance use. Screen clients, administer screening tools, connect families to treatment and benefits, transport clients, support DFS staff with education and liaison work, participate in case presentations, perform outreach, document progress, and provide crisis intervention as needed.

Provide Hep C-focused case management including referrals, care navigation, aftercare, follow-up, group and individual counseling, community outreach, recordkeeping, and occasional community supervision and driving to support clients' social determinants of health and treatment adherence.

Provide clinical oversight in an outpatient behavioral health setting: review counselor and peer caseloads, approve treatment plans/progress notes/discharges, conduct staff orientation and training, participate in case consultations and meetings, ensure electronic records meet licensure and Joint Commission standards, confer with medical staff and program manager, maintain compliance with agency procedures, and possibly carry a small caseload.

Provide community-based case management and DFS liaison services to identify substance use in families, connect clients to treatment and benefits, conduct screenings, transport clients, participate in case meetings, deliver outreach and education, and document client progress to promote child safety and reduce future harm.

Provide peer-based case management and recovery support to clients in the Drug Diversion Program. Teach coping and problem-solving skills, support goal-setting and wellness planning, facilitate community integration and job-readiness skills, make referrals, and perform duties assigned by the Peer Recovery Supervisor.

Provide clinical oversight for outpatient treatment: supervise counselors and peers, review caseloads, approve treatment plans/progress notes/discharges, conduct staff training and case consultation, maintain compliant electronic records, confer with medical staff and program manager, and carry a small caseload as needed.

Provide outreach, re-engagement, and peer-based support to individuals with substance use disorders; maintain caseload, liaise with clinical staff, run non-clinical groups, conduct community presentations, build referral partnerships, provide aftercare, and travel statewide for screenings and outreach.

Provide peer support and advocacy for individuals in recovery through outreach, in-home and community visits. Teach coping and self-care skills, model recovery, support job-readiness social skills, and ensure quality, client-directed services.

Provide intensive, recovery‑oriented post-release case management for justice-involved individuals. Conduct assessments, develop service plans, coordinate SUD/MAT, mental health, housing, employment, and medical referrals, use MI/CBT/SFT techniques, maintain documentation, and collaborate with corrections and community partners to support reintegration and recovery.

Provide nursing navigation for a Hepatitis C program: coordinate care, manage appointments, perform POC testing and phlebotomy, assist with prior authorizations, educate patients, and serve as primary patient contact to ensure treatment adherence.
