Provide community-based DFS liaison and case management services for families affected by substance use. Screen clients, administer screening tools, connect families to treatment and benefits, transport clients, support DFS staff with education and liaison work, participate in case presentations, perform outreach, document progress, and provide crisis intervention as needed.

Provide Hep C-focused case management including referrals, care navigation, aftercare, follow-up, group and individual counseling, community outreach, recordkeeping, and occasional community supervision and driving to support clients' social determinants of health and treatment adherence.

Provide clinical oversight in an outpatient behavioral health setting: review counselor and peer caseloads, approve treatment plans/progress notes/discharges, conduct staff orientation and training, participate in case consultations and meetings, ensure electronic records meet licensure and Joint Commission standards, confer with medical staff and program manager, maintain compliance with agency procedures, and possibly carry a small caseload.
