The Resource Navigator conducts outreach, participant recruitment, intake, assessments, screening, goal planning, intensive case management, referrals, service coordination, basic-needs support, transportation assistance, and follow-up for families. The role documents services in Salesforce and PEIRS, supports workshops and caregiver groups, provides child-abuse-prevention education, collaborates with multidisciplinary teams, attends training, and maintains compliance with program and grant requirements.

Provides case management and transition services to youth ages 16–21, including intake assessments, individualized service plans, crisis management, referrals, documentation, and progress monitoring. Leads and mentors staff, ensures compliance with DFPS, grant, and organizational requirements, conducts case reviews, prepares reports, coordinates services, and presents on transition programs. Requires community engagement, public speaking, travel, and collaboration with agencies and multidisciplinary teams.

Coordinates community outreach, education, and recruitment of eligible families for program services. Builds relationships with families, healthcare providers, community leaders, and service agencies; provides referrals, facilitates meetings and training, coordinates events, maintains participant records in Salesforce, and ensures compliance with agency and regulatory requirements. The role also includes supervised transportation, frequent community outreach, documentation, and travel as needed.

Supervises case managers, educators, and community health workers delivering client intake, benefits assistance, care coordination, home visits, referrals, advocacy, and health education. Oversees quality assurance, chart audits, productivity, documentation, regulatory compliance, reporting, community partnerships, and Salesforce data entry. Provides staff guidance, conducts education sessions as needed, supports program initiatives, and ensures service targets and deadlines are met. Requires occasional schedule flexibility, travel as needed, and adherence to safety and agency standards.

Provide clerical support to the program director and staff including reception, data entry, case file and database management, purchasing and budget tracking, travel coordination, community outreach, and occasional on-call client support while maintaining regulatory compliance.

Directs youth-serving programs by supervising staff, overseeing case management, budgets, contract compliance, documentation, reporting, and program outcomes. Coordinates with community partners, government agencies, employers, training providers, and multidisciplinary professionals. Guides employment preparation, placement, job coaching, and retention services for youth transitioning into adulthood. Manages staff development, audits, crisis response, financial tracking, deliverables, and policy adherence.

Assist program staff and clients with independent living preparedness through training, resource linkage, and case management support.

Lead and manage the CYD program by developing program components, ensuring contract compliance, supervising and mentoring direct service staff, overseeing budget and reporting, coordinating outreach and partnerships, maintaining documentation and safety protocols, and ensuring quality services for youth.

Provide case management and advocacy to youth transitioning from foster care. Assess needs, develop Individual Service Plans, coordinate resources and community services, de-escalate crises, maintain records in Salesforce, comply with HHS policies, attend trainings, and participate in program events including evenings/weekends.
