Conduct structured telehealth intake consultations, E/M visits, and Annual Wellness Visits to establish medical necessity for care navigation. Identify high-risk conditions and social needs, document diagnoses, risk factors, and SDOH Z-codes, and develop or approve individualized care plans. Collaborate with advocates and clinical operations to support compliant patient enrollment and ongoing care navigation.

Conduct structured telehealth intake consultations, including E/M and Annual Wellness Visits, to establish medical necessity for care navigation. Identify high-risk conditions and social needs, document diagnoses, risk factors, and SDOH Z-codes, and validate individualized care plans. Collaborate with advocates and clinical operations to enroll patients safely and compliantly. Requires an active U.S. medical license, broad multistate licensure, primary care or telehealth experience, Medicare knowledge, EMR proficiency, and bilingual Mandarin fluency.

Own Baba’s payer partnership strategy, pipeline, negotiations, executive relationships, and expansion with national and regional health plans, especially Medicare Advantage and dual-eligible populations. Structure value-based and risk-aligned agreements, scale pilots into contracts, communicate payer needs internally, and build a partnerships and account management team. The role reports to the CEO, collaborates cross-functionally, and requires travel for business development meetings and conferences.
