Coordinates patient referrals within a community health clinic by processing requests, scheduling appointments, tracking referrals, communicating with patients, insurers, and outside providers, maintaining EHR records and referral logs, and supporting quality improvement activities.

Provide clinical consultation and oversight for the CalAIM program, review intakes and care plans, advise care coordination, referral center, call center and transportation staff, identify and manage high-risk patients, and support individualized care planning under the Director of Patient Support Services.

Coordinate care for high-risk patients under CalAIM: enroll members, perform assessments, develop care plans, support medication adherence, coordinate post-hospital follow-up, maintain records, monitor metrics, and liaise with care teams and community services.
