Leads daily hospice homecare operations, staffing, clinical coordination, quality, regulatory compliance, financial performance, and team engagement. Supervises and develops interdisciplinary staff, supports complex patient cases, manages performance and productivity, coordinates education and competency requirements, and strengthens community partnerships to support referrals and census growth. Provides leadership for 24/7 on-call operations and ensures safe, consistent, patient-centered care.

Provides first-line help desk and systems support, including troubleshooting hardware, software, networks, mobile devices, printing, user accounts, and access. Maintains technology assets, documentation, and secure equipment containing PHI. Develops and delivers technology training, supports elevated technical issues, and participates in an after-hours on-call rotation across Agrace locations.

Provides hospice and palliative nursing care through patient and family assessments, home visits, symptom and pain management, education, care-plan development, documentation, and interdisciplinary coordination. Oversees delegated care by CNAs and LPNs, evaluates patient outcomes, responds to changing conditions, and supports staff and student precepting. The role includes rotating weekend, holiday, and on-call responsibilities.

Provides supervised hospice and palliative nursing care as part of Agrace’s new graduate orientation program. Assesses patients and families, documents needs, supports symptom and pain management, develops care plans, provides education, evaluates interventions, collaborates with interdisciplinary teams, and supervises delegated care under an RN. The role includes structured mentoring, mandatory education, and preparation for transition to registered nurse practice after completing the NCLEX-RN.

Provide hospice and palliative nursing assessments, determine eligibility, create and implement individualized plans of care, manage symptoms and pain, educate patients and families, consult with interdisciplinary teams, precept new staff, and fulfill travel nursing assignments across Agrace service areas.

Provide patient- and family-centered nursing care and case management in home hospice and palliative settings. Conduct assessments, create and update plans of care, deliver interventions and pain management, document in EHR, coordinate with interdisciplinary team, delegate to CNAs/LPNs, and participate in meetings and precepting.

Lead strategic and operational management of the Netsmart MyUnity EHR and related clinical systems. Partner with clinical, IT, and vendor teams to optimize workflows, oversee upgrades, testing, ticket resolution, and after-hours support. Manage clinical IT projects, training, change management, staff supervision, compliance, budgeting, and continuous improvement to support high-quality, patient-centered care.
