Provide administrative support to homecare and resource clinical teams: schedule staff, review timesheets, coordinate and facilitate IDT and general meetings, track recertifications and DME, assist with EHR/Epic tasks, run reports, process timekeeping, support onboarding, and ensure regulatory compliance.

New Graduate Nurse position providing supervised patient-centered hospice and palliative care. Under RN supervision, perform assessments, assist with symptom management and care planning, educate patients/families, document interventions, delegate LPN/CNA tasks as directed, and participate in a structured orientation and mentorship program.

Provide patient- and family-centered hospice and palliative nursing care through assessments, individualized care plans, symptom and pain management, education, documentation in the EHR, coordination with the interdisciplinary team, delegating to CNAs/LPNs, and occasional precepting. Includes on-call, weekend and holiday rotations.
