Provides supervised hospice and palliative nursing care as part of Agrace’s new graduate orientation program. Assesses patients and families, documents needs, supports symptom and pain management, develops care plans, provides education, evaluates interventions, collaborates with interdisciplinary teams, and supervises delegated care under an RN. The role includes structured mentoring, mandatory education, and preparation for transition to registered nurse practice after completing the NCLEX-RN.

Provide hospice and palliative nursing assessments, determine eligibility, create and implement individualized plans of care, manage symptoms and pain, educate patients and families, consult with interdisciplinary teams, precept new staff, and fulfill travel nursing assignments across Agrace service areas.

Provide patient- and family-centered nursing care and case management in home hospice and palliative settings. Conduct assessments, create and update plans of care, deliver interventions and pain management, document in EHR, coordinate with interdisciplinary team, delegate to CNAs/LPNs, and participate in meetings and precepting.
