Teams at ADAPT of Hudson Valley

Supports daily operations of a day habilitation program serving individuals with developmental disabilities. Develops individualized habilitation and behavior plans, ensures participant safety and regulatory compliance, coordinates with therapeutic, nursing, family, and agency teams, supervises DSP staff, manages staffing, medication administration oversight, billing documentation, funds, trainings, fire safety, and agency vehicles. Requires community partnership development, strong communication, sound judgment, computer proficiency, and travel among agency sites.

Oversee multiple group homes for people with disabilities: ensure safety, regulatory compliance, budgeting, staff supervision, incident investigation, plan development, admissions/discharges, training, and on-call coverage.

Provide case management and social services in a residential setting: develop and monitor individualized service/treatment plans, coordinate and document services, supervise residence staff, liaise with families and community agencies, ensure benefits and placements, and maintain confidential records.
