Provide community-based case coordination and rehabilitation services for supportive housing residents. Conduct home visits, assess psychosocial and functional needs, develop and update individualized service plans, coordinate psychiatric, medical, and entitlement services, monitor medication, deliver skill-building and crisis intervention, maintain records and incident reports, and support housing maintenance and transitions to independent living.

Provide senior-level case coordination and direct community rehabilitation services for supportive housing residents. Conduct home visits, assess psychosocial and functional needs, develop and update individualized service/support plans, coordinate psychiatric/medical/substance abuse services, monitor medication, facilitate crisis intervention, supervise staff in director’s absence, ensure program compliance with NYS OMH regulations, oversee apartment maintenance, and assist the Program Director with operations, quality assurance, and staff mentoring.

Provide front desk coverage at a supportive housing residence: monitor entry/egress, distribute mail, take messages, conduct rounds, document incidents, assist in emergencies and medication self-administration, and cover other sites as needed.

Provides administrative and fiscal support for a supportive housing program: collects rents and fees, maintains client finance ledgers and petty cash, handles banking and bookkeeping, tracks move-ins/outs and leases in RealPage, prepares monthly reports and billing, assists with staff scheduling and building maintenance, manages office supplies and mail, and performs other duties as assigned.

Provide community-based case coordination and rehabilitation services for adults with serious mental illness, including home visits, psychosocial assessments, individualized service/support plans, medication monitoring, crisis intervention, housing support, documentation, and referrals. Attend trainings, cover emergency phone rotations, and support transitions to independent living.
