Provide community-based case coordination and rehabilitation services for adults with serious mental illness. Conduct home visits, assess psychosocial and functional needs, develop and update individualized Service/Support Plans, connect clients to psychiatric, medical, substance use, housing and entitlement services, monitor medications, provide crisis intervention, document progress and incidents, support transitions to independent living, and staff rotating emergency phone coverage.

Provide case coordination and direct community rehabilitation services for supportive housing residents. Conduct assessments, develop and update individualized service plans, coordinate psychiatric/substance abuse/medical care, monitor medications, perform crisis intervention, supervise staff coverage, ensure NYS OMH compliance, and assist the Program Director with program operations and quality assurance.

Provide community-based case coordination and rehabilitation services for adults with serious mental illness, including home visits, psychosocial assessments, individualized service/support plans, medication monitoring, crisis intervention, housing support, documentation, and referrals. Attend trainings, cover emergency phone rotations, and support transitions to independent living.
