Provides case coordination, community rehabilitation, supportive housing, crisis intervention, and care coordination for adults with serious mental illness. Conducts home visits, assessments, service planning, medication and health monitoring, skills training, benefits referrals, documentation, and transition planning. Maintains client safety, apartment conditions, and compliance records while coordinating with psychiatric providers and treatment teams.

Monitors front desk access, resident and guest entry, mail distribution, telephone messages, building rounds, and emergency response at a supportive housing residence. Maintains detailed logs, documents medication self-administration, reports incidents and behavioral changes, assists with meal plans, supports crisis intervention and evacuation, and provides coverage at other residential locations as needed. Requires a high school diploma, sound judgment, responsibility, and the ability to work respectfully with residents, staff, and visitors.

Directs the overall operation of a mental health residential program and facility. Oversees service delivery, regulatory compliance, staffing, training, admissions, discharges, clinical reviews, documentation, safety programs, and case management. Manages building upkeep, vendors, rent arrears, reports, employee schedules, and emergency on-call responsibilities. Promotes evidence-based rehabilitation and recovery practices, coordinates community resources, and evaluates service quality.
