Provides case management, life-skills education, assessments, transition planning, referrals, and ongoing support to homeless youth and young adults in extended foster care. Coordinates community resources, housing transitions, group programming, documentation, and aftercare while applying trauma-informed, solution-focused practices. Requires collaboration with agencies, client advocacy, crisis response, confidentiality, and travel.

Provides intensive case management, crisis intervention, therapeutic support, advocacy, and skill-building for kinship families and children. Conducts assessments, develops service plans, coordinates community resources, supports caregiver groups, performs home visits, and collaborates with schools, courts, DFPS, therapists, CASA, and other providers. Participates in an after-hours on-call rotation.

Coordinates fundraising, community engagement, donor stewardship, volunteer activities, in-kind donations, and internal and external events. Supports donor acquisition and renewal, mailings, digital newsletters, database records, agency visibility, and development team initiatives. The role requires strong organization, interpersonal communication, customer service, time management, Microsoft Office proficiency, confidentiality, and occasional evening and weekend event support.

Supervises overnight residential youth-care staff, ensuring staffing coverage, treatment-plan and TBRI implementation, safety, documentation, medication protocols, and regulatory compliance. Provides on-floor coaching, crisis support, performance management, shift coordination, and staff development. Collaborates with clinical and case-management teams, conducts facility and documentation audits, supports youth routines and life-skills development, and provides after-hours or emergency coverage as needed.

Manages transition programs serving young adults, including foster-care youth and community-program participants. Supervises case managers and mentors, oversees housing settings, coordinates referrals and intakes, ensures grant and contract compliance, monitors client services and records, manages budgets, develops community partnerships, tracks outcomes, and supports trauma-informed programming and on-call services.
