Provide light housekeeping, laundry, meal preparation, grocery shopping, appointment assistance, and companionship to help clients remain independent at home. Report changes to Clinical Manager and follow documentation practices.

Coordinate scheduling to meet patient needs, communicate professionally with employees, patients, and responsible parties, maintain HIPAA confidentiality, and provide quality customer service and data entry support for the home care team.

Provide direct nursing care in patients' homes under RN Case Manager direction, complete visits, support nursing and home health aide services, and perform phlebotomy, wound care, catheter care, medication management, and patient education as needed.
