Provides direct nursing care during home health visits under an RN Case Manager’s direction. Responsibilities include completing patient visits, supporting nursing and home health aide services, and performing phlebotomy, wound care, catheter care, medication management, and patient education. The nurse manages a flexible schedule and must maintain Minnesota LPN licensure, BLS certification, transportation, and the ability to transfer patients weighing up to 50 pounds.

Provides in-home support to help clients remain independent, including light housekeeping, laundry, meal preparation, grocery shopping, appointment assistance, and companionship. Reports client changes or concerns to the Clinical Manager. The role requires compassion, communication skills, English fluency, a valid driver’s license, reliable transportation, and the ability to lift up to 25 pounds.

Provide direct nursing care in patients' homes under RN Case Manager direction. Complete visits, perform phlebotomy, wound and catheter care, medication management, patient education, and oversee home health aide services. Manage your own schedule and document care, travel between patient homes, and maintain patient safety and experience.
