Provide comprehensive case management for youth: assessments, treatment and transition planning, service coordination, documentation (treatment plans, court reports, FASPs), family visitation facilitation, monitor services, ensure compliance, and communicate with supervisors and providers to support residents' needs.

Assist Case Manager with case management tasks: contacting sponsors/families/legal representatives, collecting and uploading documents, preparing discharge packages, coordinating services and referrals, participating in staffing and rotations, conducting occasional home visits, and completing required ORR/agency trainings.

Coordinate and deliver independent living and aftercare life-skills services for youth in group homes and foster care, including assessments, goal-setting, discharge planning, documentation, community resource linkage, housing and ETV support, group/individual training, stipend management, and at least six months post-discharge follow-up.

Provide therapeutic social work services to children, foster families, and natural families in TFFC programming. Coordinate community, school, medical, and court-related services; complete and maintain LDSS/ACS paperwork; facilitate family visits, discharge planning, foster parent support and training; attend courts, team meetings, and supervisions; document case activity and work toward family reunification.

Provide individual and group mental health counseling, crisis intervention, and family/teacher support in two Bronx community schools. Maintain clinical records, lead workshops and trainings, participate in treatment teams, perform home visits and referrals, and support socio-emotional learning initiatives.

Provide casework services to children in foster care and their families to achieve permanency: conduct monthly and weekly contacts, coordinate services and visitation, maintain contemporaneous records, prepare FASPs and court reports, represent the agency in court, participate in family conferences and mandated trainings.

Provide individual and group counseling, assessments, crisis intervention, and clinical guidance for youth and staff. Maintain clinical records, perform intakes, participate in treatment teams, and deliver trainings to support therapeutic program goals.

Assess needs of unaccompanied children, develop and adapt Individual Service Plans, screen for human trafficking, coordinate inter-agency services, manage safe timely discharges/unifications, document case files per ORR and state standards, perform weekly face-to-face contacts and home visits, complete required trainings, and serve as a mandated reporter.

Provide individual and group counseling, diagnostic assessments, crisis intervention, and clinical guidance to case managers and youth staff; maintain records, complete ORR assessments, attend treatment team meetings, and support program therapeutic requirements.

Provide casework and service coordination for children in foster care and their families to achieve permanency. Conduct regular contacts with children and parents, manage visitation plans, coordinate medical/clinical/educational services, maintain timely case records and reports, represent the agency in court, participate in mandated trainings, and perform required administrative duties.

Supervise Youth Specialists and daily unit operations for Unaccompanied Alien Children, ensuring safety, policy compliance (ORR), staff training, scheduling, crisis intervention, and coordination of services and supplies.

Provide clinical assessments and individual, family, and group therapy for children and adolescents in foster care and residential placements. Participate in treatment team meetings, consult with social work and community providers, document evaluations and treatment, and apply evidence-based practices for complex trauma.
