Provide comprehensive case management for youth: assessments, treatment and transition planning, service coordination, documentation (treatment plans, court reports, FASPs), family visitation facilitation, monitor services, ensure compliance, and communicate with supervisors and providers to support residents' needs.

Assist Case Manager with case management tasks: contacting sponsors/families/legal representatives, collecting and uploading documents, preparing discharge packages, coordinating services and referrals, participating in staffing and rotations, conducting occasional home visits, and completing required ORR/agency trainings.

Coordinate and deliver independent living and aftercare life-skills services for youth in group homes and foster care, including assessments, goal-setting, discharge planning, documentation, community resource linkage, housing and ETV support, group/individual training, stipend management, and at least six months post-discharge follow-up.
