As the Team Lead Social Worker, you will promote the achievement of optimal clinical and resource outcomes in a defined population, provide resources when the Director is unavailable, serve as the key information and education resource for the inter-professional team, and works/ to assist the team in achieving department and organizational goals.

Job Responsibilities:

  • Promotes the achievement of optimal clinical and resource outcomes.
  • Facilitates the collaborative management of patient across the continuum, proactively identifying and effectively addressing barriers for defined populations.
  • Works with leadership to align goals and visions.
  • Supports and acts as a role model for the compliance of team members ensuring regulatory requirements are met.
  • Other duties as assigned.

Education:

  • Graduate from an accredited school of Social Work.
  • Master's degree required.

Certifications:

  • Current and valid license to practice Social Work (LSW) in the State of Ohio.
  • American Heart Association Basic Life Support (BLS) required.
  • ACM (accredited case manager) certification preferred.

Competencies:

  • Requires clinical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.

Work Experience:

  • Three years' experience in Care Management.

Physical Requirements:

  • Requires full range of motion, manual and finger dexterity and eye-hand coordination.
  • Requires standing and walking for extensive periods of time.
  • Requires normal or connected hearing and vision to normal range.
  • Sedentary work exerting up to 10 pounds of force (Occasionally: active or condition exists up to 1/3 of the time) and/or a negligible amount of force (Frequently: activity or condition exists from 1/3-2/3 of the time to lift, carry, push, pull or otherwise move objects, including the human body.

Personal Protective Equipment:

  • Follows Standard Precautions using personal protective equipment (PPE) as required.
