About the role:

As a Community Outreach Specialist, you are responsible for meeting with members who are eligible for Cityblock, and engaging with them around their needs. This initial series of conversations with members begin the intake process, and are the stepping-off point from which a member is connected to a Community Health Partner to develop a long-term relationship.  Until this connection is fully made, you are the member’s point person, available and accessible to assist with whatever may be needed, to troubleshoot, to educate, and to be a listening ear.  You work alongside, and are supported by, each member’s care team. This role is field-based.

Responsibilities:

  • Locate and meet with eligible members in their homes, neighborhoods, and the Cityblock health hub; nearly all of your work is done in the field, seeing members for scheduled and unscheduled visits. This includes traveling to their homes, accessing their space, sitting with them to explain Cityblock services and consenting eligible members to the program. 
  • Reviewing pre-populated demographic information and documenting accurate information and emergency contacts within Commons.
  • Leveraging available data from Commons to learn more about potential members and incorporate adjusted strategy into your outreach process based on that data/information. 
  • Completing the consent process for eligible members fully and accurately, including obtaining written consent per member. 
  • Engage with and serve as the initial contact and representative of Cityblock, responsible for initiating the member intake process and successful and smooth handoff to the care team to ensure positive experience for members and careteam colleagues. 
  • Effectively communicate urgent needs and other pertinent information in an accurate and timely manner to care teams.
  • Working daily within Commons to collect data, organize information, track tasks, monitor performance, and communicate with staff, patients, and family members. 
  • Providing feedback to the product development team, and, over time, becoming part of a super-user group to assist in onboarding and supporting others

Requirements:

  • 1+ year of experience 
  • Demonstrated ability to conduct secondary research
  • Proficiency in tech platforms: Google Suite, EHR systems
  • Comfortable providing telephonic, written, digital, and face-to-face outreach, and running group outreach/events
  • Strong customer service mindset and a passion for Cityblock's mission and members
