Job Summary:   

As an essential member of a multi-disciplinary team, the Wraparound Facilitator provides a strength-based and personalized High-Fidelity Wraparound approach to service delivery for youth with complex needs and their families. Ensures the values and steps of the wraparound process are implemented to the highest possible level of fidelity according to best practices while allowing for local individualization of the process. Helps the family develop a pro-social narrative as a part of the strengths, needs and culture discovery. Fosters the development of a partnership between the family and the team members in order to identify and overcome the challenges needed to change their lives. Works with individuals and their families in various settings, including but not limited to: outpatient, home, or community-based environments.  


Essential Functions: 

  • Ensures all phases and activities of the wraparound process are done to high fidelity through frequent connections with professional team members and/or the family. 

  • Conducts functional assessments for youth and/or the family. 

  • Demonstrates ongoing engagement through education. 

  • Facilitates wraparound team meetings resulting in the identification of a family-engaged Plan of Care. 

  • Collaborates with team members to create a personalized Crisis Prevention Plan for the family. Secures support and crisis/emergency services for the youth/family utilizing existing crisis supports and resources. 

  • Provides follow-up on service requests related to the identified Plan of Care. Navigates treatment needs and monitors the provision of quality services provided to the family.  

  • Actively engages in training and/or skill development in order to work with families and children/youth using a Wraparound process. Assists in the identification of personal, community and professional resources to meet client needs and to "wrap" services around the child/youth and family.  

  • Responsible for the completion of all the necessary paperwork (i.e., court letters, service recommendations, contact progress notes, consent forms, data requirements to include CCAR and NOMS, etc.) and maintains the family files according to requirements including for effort/outcome data analysis and quality improvement purposes. 


Knowledge/Skills/Abilities: 

  • Bachelor’s Degree in behavioral health or healthcare related field, required. 

  • General understanding of Mental Health and/or Substance Abuse services 

  • Working knowledge of local community resources. 

  • Experience working therapeutically with specified population, to include using various treatment modalities appropriate for specified population, utliziing strength-based therapeutic approaches. Experience working in a team environment.   

  • Strong written and oral communication skills with the ability to accurately and professionally implement and document services rendered. 

  • Ability to easily build rapport with clients and other team members. 

  • Proficient computer skills, to include Microsoft Suite, with the ability to type 25+ w.p.m. for the input and output of client information, using electronic medical records.
