POSITION TITLE: LPN ASSISTANT TO THE DIRECTOR OF WELLNESS
DEPARTMENT: Clinical Services
REPORTS TO: Director of Wellness Nurse
POSITION SUMMARY:
- The essential functions of the job for the Wellness Nurse, requires, and may be amended from time to time at the sole discretion of Viva Senior Living to include, but are not limited to the following:
- Comply with standards of business conduct in accordance with federal, state and local health and regulatory standards and guidelines, as applicable.
- Must possess a valid LPN license issued by the State
- Must possess, at a minimum, a high school diploma or a GED
- Must be able to work well with peers in a congenial work environment, and above all, maintain the utmost compassion and dignity to the residents and adhere to the Resident Bill of Rights.
- In exigent circumstances, the employee may be subject to call back during emergency conditions (e.g., severe weather, evacuation, temporary staffing shortages, etc.).
- Must be able to work an on-call schedule and participate in some of the community events that take place after hours and on weekends.
- Must adhere to Viva Senior Living’s attendance policies and follow the terms and conditions of employment as set forth in the Employee Handbook.
- Must be able to read, write, speak and understand English.
- Must maintain the highest standards in caring and servicing the needs of the residents and residents' family members and loves ones.
- Must adhere to all facility policies, practices and procedures.
- This is a salaried, leadership position that reports to the Director of Wellness Nurse.
RESPONSIBILITIES/ACCOUNTABILITIES
- The Wellness Nurse is responsible to provide care, treatment, medication administration to assigned residents, and to continually monitor and assess the residents’ condition and report in accordance with plans of care, established medical practices, and all applicable laws and regulations, and Viva’s policy and procedure.
- Assists with oversight of ancillary care staff in the absence of the Wellness Director or Assistant Wellness Director
- Provide care to al residents based on plans of care and environmental and spiritual needs
- Medication assistance and/or administration with adherence to the Five Rights of Medication Administration
- Reordering and receipt of medications
- Provide personal care and ADL assistance
- Supervision and Scheduling of Care staff
- Maintains shift to shift report
- Provides emergency assistance to residents with appropriate follow-up
- Obtains and transcribes physician orders
- Maintains case management documentation
- Obtains and maintains a Class E transportation driver's license
- Shares the duties of medical transportation with other management staff
- Monitors scheduling for budget adherance through daily and weekly reporting
- Handles disciplinary actions professionally and with dignity to all involved
- Attends, provides, and documents all education provided to care staff
- All other duties as required to maintain a professional, caring, safe and efficiently managed nursing department in the presence and absence of the Wellness Director as explained in the employee handbook and under the guidance of the Executive Director.

- Must be able to move intermittently throughout the workday.
- Must be able to cope with the mental and emotional stress of the position.
- Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
- Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
- Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, agitated residents
- Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
- May be necessary to assist in the evacuation of residents during emergency situations.
Skills Required
- Valid LPN license issued by the State
- High school diploma or GED
- Ability to read, write, speak, and understand English
- Ability to work on-call, evenings, and weekends and participate in community events
- Annual verification of negative TB skin test
- Proof of full COVID-19 vaccination (medical/religious exceptions allowed)
- Ability to push, pull, move, lift a minimum of 25 pounds to 5 feet and carry 50 feet
- Obtain and maintain a Class E transportation driver's license
- Medication administration skills and adherence to Five Rights of Medication Administration
- Ability to work compassionately with ill, disabled, elderly, and agitated residents
