Position Summary

The Veterans Community Facilitator is responsible for supporting the Veterans Administration (VA) Community Coordinator within LMM’s Veterans Community at the Men's Shelter. This position is supervised by the Community Coordinator and assists with case management, housing navigation, and connecting Veterans to supportive services and community resources. The goal of this role is to collaborate with the VA and other community partners to help end Veteran homelessness by making homelessness rare, brief, and non-recurring. The Veterans Community Facilitator supports Veterans experiencing homelessness in achieving housing, increasing income, life skills, and enhancing self-determination. This position also works closely with the VA to identify and engage individuals who are most appropriate for program participation.


Some of the duties include:

  • Collaborate with residents and other relevant community partners to devise in the formulation of exit plans that will move participants toward rapid exit of homelessness.
  • Assist residents with obtaining essential documents (ID, birth certificate, Social Security card, DD214, etc.)
  • Connect and refer residents to community resources and partner agencies for essential support services.
  • Arrange transportation to critical destinations (e.g. housing, medical, employment, etc.)
  • Maintain participant data and statistical information in the client database system
  • Maintain quarterly and annual reporting for VA partnership as appropriate
  • Assist in annual and unscheduled VA site visits/audits
  • Maintain regular scheduled meetings with VA staff for individual resident/staffing updates
  • Assist in managing Shelter emergencies in adherence to program policies, prioritizing the safety of both clients and staff
  • Conduct routine shelter area inspections to maintain quality standards
  • Engage in bi-weekly case reviews with residents to assess progress toward individualized goals and action plans
  • Monitor resident compliance with shelter policies, program rules, and expectations, addressing concerns as appropriate
  • Coordinate with the VA staff to provide regular group sessions
  • Actively participate in trainings and professional development opportunities provided by the agency, the VA, and other relevant community partners

Qualifications

Our ideal candidate has a High School Diploma or GED; Associate’s/Bachelor’s Degree preferred. Some experience working with people experiencing homelessness, veterans, people with substance use disorders, people with re-entry backgrounds, and/or other relevant social service populations, preferred. A Valid Ohio Driver’s license and proof of insurance, required
