Pay Rate: $3120 to $3200 Weekly Gross pay based on a 40-hr work week (Includes Non-taxable
travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage
Reimbursement: $0.49/mile for distance
covered between patient home appointments

Position Requirements:
- Physical Therapist: 1-2 years - Required
- Recent/Current Home Health PT experience: 1 year within the last 3 years - Required
- OASIS experience - Required
- Home Care Home Base (HCHB)
charting - Preferred
- Must have valid Driver’s License
and Personal Auto Insurance
Education, License
and Certification Requirements:
- Degree in Physical Therapy - Required
- PT License for State applying or Compact if applicable - Required
- BLS Certification from AHA or
American Red Cross - Required
Job Description:
- Provide Physical Therapy services for Home Health Patients
- Consult with the physician and an interdisciplinary team in the
development of a plan of treatment
- Utilize skills in patient/client evaluation and treatment, professional
consultations to achieve maximum patient/client independence in accordance with
the individual's functional level and potential
- Plans and prepares treatment plan based on evaluation of patient data
- Provide in-home PT treatment programs to assist patients in restoring
function, relieving pain, and preventing disability
- Allow patients and their families to maintain dignity and
independence while receiving safe, effective rehabilitation in the security of
their home
- Drive to patient homes in the assigned territories on a daily basis.
- Complete charting and patient documentation in timely manner as per
client policies and procedures
- Territories: Girdwood to Peter's Creek
Skills Required
- 1-2 years experience as a Physical Therapist
- 1 year recent/current Home Health PT experience within the last 3 years
- OASIS experience
- Home Care Home Base (HCHB) charting
- Valid Driver's License and Personal Auto Insurance
- Degree in Physical Therapy
- PT license for the state applying or Compact if applicable
- BLS certification from AHA or American Red Cross
