Job Summary

The Texas Medical Center is seeking an experienced Transplant Coordinator – Pre-Liver to join a dedicated, collaborative team. This role manages candidate evaluations, listing, and care coordination for patients awaiting liver transplantation, working within a balanced team structure and an manageable patient volume.
Role Overview
• Team Structure: 3 Pre-Liver Coordinators, 3 Post-Liver Coordinators
• Patient Load: Low Nurse-to-Patient ratio of 5–8 patients per coordinator
• Schedule: Monday–Friday, 8:00 AM – 5:00 PM (On-call duties begin after a 6–8 month orientation period)
Key Responsibilities
• Oversee comprehensive pre-liver transplant clinical evaluations, patient education, and UNOS listing processes.
• Serve as the primary point of contact for patients, families, and multidisciplinary clinical teams.
• Utilize Epic EMR for efficient patient tracking, precise documentation, and seamless navigation.
• Drive proactive care management by taking initiative to support team workflow and patient progress.
Qualifications & Requirements
• Direct experience as a Transplant Coordinator across solid organ specialties (Liver, Heart, Lung, or Kidney) is preferred.
• Transferable Experience: Strong background as a Liver Transplant ICU Registered Nurse is accepted (2+ years)
• Core Competencies: Exceptional communication skills, high clinical adaptability/flexibility, and a strong drive to initiate support across the team.
• Technical Skills: High proficiency with Epic EMR navigation and clinical documentation (strongly preferred).

Position develops, implements, and monitors transplant program processes to improve patient care; responsible for performing initial assessment of transplant evaluation patients, assisting the patient for transplant and providing follow-up post transplant. May be asked to assist in supervising the activities of all department employees in consultation with the department Director and/ or Transplant Manager.Job Description

Minimum Qualifications

Education: Bachelors of Science in Nursing

Licenses/Certifications: Current State of Texas license to practice professional nursing and Certified Transplant Coordinator (CCTC) or Certified Transplant Nurse (CCTN)

Experience / Knowledge / Skills:

• Five (5) years clinical experience in transplant specifically
• Knowledge of transplant regulatory mandates through CMS and UNOS
• Actively participates in local or national chapter of ITNS, NATCO or other equivalent organizations
• Ability to promote teamwork, build effective relationships, and lead/facilitate work groups
• Strong customer service skills
• Ability to develop and direct transplant program processes
• Knowledge of transplant and hospital computer systems

Principal  Accountabilities

• Develops and maintains a communication system which assures effective flow of information to and from staff. Serves as a resource and role model for competent professional practice by educating, mentoring, and supporting transplant staff and peers.
• Seeks new ways of improving operational transplant services. Works with others to continually identify areas for improvement.
• Performs initial assessment of patients’ biophysical and psychosocial status and ensures appropriate actions are taken in relation to the nursing process (assessment, planning, education, intervention, and evaluation), noting all abnormal results and reviewing with transplant surgeon in a timely manner; documents the nursing process, including problem list, plan of care, and patient education.
• Provides appropriate pre/post-transplant interventions within limits of licensure and/or scope of practice.
• Evaluates and collaborates with other healthcare providers to achieve outcomes; plans, coordinates and organizes the discharge plan with inpatient nursing staff to assure continuity of care.
• Directs and provides guidance to staff, case managers, and physicians, ensuring patient care delivery to established standards of practice.
• Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
• Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
