Key Position Details:

  • This position supports the Advanced Heart Failure Clinic at Abbott Northwestern Hospital.
  • 1.0 FTE (80 hours per two week pay period)
  • 8-hour day shifts
  • No weekends

Job Description:

Uses specialized clinical knowledge and expertise, to manage care for those patients with organ failure and transplant candidates throughout the continuum and in a variety of settings. This can include inpatient units, outpatient clinic, home and community. Serves as authoritative resource to staff and multi-disciplinary team. Establishes policy, procedure, standards and systems of care for the patient population served.
Principle Responsibilities

  • Coordinates assessment, referral and intake of patients entering program
    • Performs initial assessment of patient according to program protocols.
    • Collaborates with physician and/or Nurse Practitioner/Physician Assistant provider to determine additional diagnostic testing and follow up intervals.
    • Provides education to patient and family regarding diagnosis, post hospital self-care, medications and medical therapies.
  • Coordinates care of patient in the clinic setting
    • Reviews medical record, patient orders and schedule prior to visit.
    • Assesses patient status during clinic visit.
    • Provides after-visit instructions based on provider orders and plan of care.
  • Coordinates care during hospital admission
    • Monitors patient progress.
    • Serves as resource to patient, family and staff for information sharing and question clarification.
    • Facilitates interdisciplinary discharge planning for short and long term follow-up.
  • Organizes systems of care for the patient population served
    • Develops, implements, and maintains policies, procedures and systems of care.
    • Facilitates ongoing continuous quality improvement for the program.
    • Serves as resource providers, staff and referring physician offices regarding patient care.
    • Participates in education for hospital staff.
  • Other duties as assigned.

Required Qualifications

  • Bachelor's degree graduate of accredited School of Nursing
  • 2 to 5 years of clinical management of patients with organ failure/transplant candidates

Preferred Qualifications

  • Master's degree

Licenses/Certifications

  • BLS Tier 1 - Basic Life Support - Multisource required upon hire
  • Minnesota Registered Nurse license current upon hire required
  • Certification in clinical specialty dependent on what patient population served upon hire preferred

Physical Demands

  • Light Work:
  • Lifting weight up to 20 lbs. occasionally, up to 10 lbs. frequently

Pay Range

Pay Range: $44.64 to $61.76 per hour

The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
