Transition Planning & Coordination
- Develop
and manage transition plans, timelines, and milestones.
- Coordinate
transition activities between internal teams, vendors, customers, and
stakeholders.
- Ensure
all transition deliverables are completed as per project schedule.
Project & Service Transition Management
- Support
onboarding/offboarding of IT services, applications, or infrastructure.
- Manage
service transition activities aligned with ITIL best practices.
- Assist
in transitioning projects from implementation to operations/support teams.
Stakeholder Management
- Act
as a single point of contact during transition activities.
- Conduct
transition meetings, status calls, and governance reviews.
- Provide
regular updates to leadership, customers, and project teams.
Documentation & Knowledge Transfer
- Ensure
all operational documents, SOPs, runbooks, and process documents are
updated.
- Coordinate
knowledge transfer sessions between project and support teams.
- Maintain
transition trackers, risk logs, and issue registers.
Risk & Issue Management
- Identify
transition risks, dependencies, and potential service impacts.
- Track
and escalate issues affecting transition timelines or service quality.
- Implement
mitigation and contingency plans.
Service Readiness & Compliance
- Verify
operational readiness before go-live or handover.
- Ensure
SLA, KPI, and compliance requirements are met.
- Coordinate
UAT, signoffs, and acceptance criteria completion.
Vendor & Resource Coordination
- Coordinate
with third-party vendors and service providers.
- Track
resource allocation and transition support requirements.
- Ensure
smooth collaboration between cross-functional teams.
Reporting & Governance
- Prepare
transition status reports, dashboards, and metrics.
- Monitor
progress against timelines and service objectives.
- Support
audit and governance requirements during transitions.
