Under the direction of the Clinical Manager, the Therapeutic Case Manager provides direct services to children, allowing easy access, timely response, and flexibility to deliver skill building services in the community. 

MAJOR RESPONSIBILITIES:

DIRECT SERVICES: Provides access to community services and activities to children with the intent of teaching, practicing, and mastering skills to decrease difficulty and increase success in the community. Services are to be provided in

groups or individually. 

CLINICAL RECORD KEEPING: Initiates and maintains, in a timely and thorough manner, the wide variety of record keeping and administrative documentation required to account for services provided to clients – including, but not limited to, treatment plans, progress notes, CANS, and timesheets. Follows all UCS record-keeping procedures and performs other duties as necessary. The case manager is responsible for the chart management of clients that are receiving therapeutic, not clinical, services through UCS.

ADMINISTRATIVE: Attends all UCS and community meetings necessary to promote coordination and communication. 

* In addition to the above, performs other duties as assigned.

JOB QUALIFICATIONS

  • High School diploma required. 
  • Bachelor’s degree in Psychology, Social Work, or related field preferred. 
  • Experience in case management, advocacy, outreach, assessment, crisis intervention, and supportive counseling necessary as well as strong ability to communicate clearly.
  • Valid Driver’s License
