Tenant Coordinator – Part-Time

Hourly Rate: $26.52
Schedule: Tuesday - Friday: 9:30am - 5:00pm

Position Summary:
The Tenant Coordinator will provide comprehensive social services to residents in a newly constructed Senior Affordable Rental Apartment (SARA) development. This includes older adults aged 60+, some of whom were previously homeless. The Coordinator will work collaboratively with the Program Director and Social Work Supervisor to promote housing stability, facilitate aging in place, and build a positive, inclusive community culture through programming and support.
Key Responsibilities:
- Assess and develop individual care plans, coordinate case management needs, work with collateral providers and natural supports to build networks of supports, and coordinate care plan meetings as necessary.
- Regular contact with clients and ongoing coordination with all referred service providers.
- Develop and facilitate counseling sessions with clients as needed.
- Provide advocacy and resource linkages as required.
- Create curriculum and training to educate building staff on the unique needs of the target population.
- Promote program-wide community and culture building activities including planning events, celebrations, and activities to contribute to an inclusive living environment. (i.e., presentations/update on Medicaid, Medicare, and other insurances, home care, training exercises, shopping trips, art and crafts, tenant appreciation day and holiday parties/lunches, fire drills if applicable, etc.)
- Written reports as required by program standards as well as timely, accurate record keeping and data collection.
- Provide customer service for tenants, including applying for benefits and entitlements, healthcare coordination, assist in the completion of rental money orders/checks, ensure tenant problems and members of the team have addressed concerns in a timely manner.
- Completion of Initial Intake, assessment, and file consent form in a timely manner.
- Completion of updated assessments when any medical or social changes take place.
- Conduct periodic home visits/wellness checks for tenants with special needs or circumstances.
- Maintain daily case assistance log.
- Completion of detailed progress notes in a timely manner.
- Completion of detailed monthly reports for HPD in a timely manner.
- Assist Supervisor and/or Program Director as needed.
- Bachelor’s Degree in Social Work or a related field, OR at least two years of college with demonstrated experience working with aging populations
- Knowledgeable in public benefits and entitlements for low-income seniors
- Proficient in Microsoft Office
- Strong interpersonal, organizational, and problem-solving skills
- Detail-oriented, dependable, and capable of working both independently and collaboratively
- Bilingual preferred (Spanish, Chinese, or Korean)
Skills Required
