- Develops and implements customized improvement plans for individual practices.
- Evaluates performance data to prioritize interventions.
- Partners with medical group leaders to optimize EHR use and ensure practice success.
- Supports process improvement and transformation for practices.
- Implements evidence-based care models.
- Assists with EHR optimization.
- Assesses workflows, identifies gaps, defines necessary changes, and develops future workflows.
- Conducts improvement audits and reports findings to the manager and team.
- Monitors data and outcome measures to evaluate user adoption of systems and processes.
- Leverages data and analytics to identify areas needing improvement.
- Collaborates closely with reporting teams to ensure accurate and timely data collection.
- Analyzes performance data by region, medical group, specialty, practice, and provider to promote optimal outcomes.

Pay Range:
