Job Summary:
The Team Lead, Strategic Partnerships is responsible to monitor work allocation to maximize output and effectiveness of staff.
Essential Functions:
- Provide daily oversight of scheduling and attendance for team
- Oversee the delivery of the Strategic Partnerships by direct reports that are assigned
- Ensure that all elements of the Strategic Partnerships (assessments, surveys, member engagement, and data tracking) are implemented effectively and that the elements remain responsive to the member’s needs
- Responsible for gathering business intelligence and managing research and analysis on a variety of social determinant of health (SDoH) related topics, including but not limited to, food, education, workforce development, health equity, and specialized populations
- Understand the correlation between SDoH and Stars, HEDIS, and NCQA measures to develop interventions and solutions for improved member outcomes
- In collaboration with CareSource University and internal teams, drive Population Health-wide training strategy through identification, growth, and tracking mechanisms
- Conducts needs assessments to identify technical solutions that address market-specific knowledge, skill, or other gaps for member population(s) and programs
- Establish Best Practices to maximize positive, consistent consumer engagement and experience
- Collaborate, plan and design Population Health training to improve staff awareness, understanding, and engagement
- Work with data analytics and research team on SDoH pilot development and deployment
- Cultivate state and community partnerships to improve our research and ability to model industry Best Practices
- Arrange and coordinate services with community leaders; partner to maximize utilization of available resources and support groups
- Monitor performance of staff members through ongoing audit process
- Provide ongoing training, mentoring, coaching and disciplinary action as needed; conduct regularly scheduled one-on-one meetings with all team members
- Coordinate team-specific orientation and training
- Monitor the accuracy and timeliness of documentation entered by direct reports (contacts, events, partner & member outreach, case notes, etc) in electronic record according to established guidelines
- Mentor and precept new staff; provide on-going training, coaching and disciplinary action as needed
- Network with other departments to gather and/or share information
- Perform other job-related instructions as requested
Education and Experience:
- Bachelor’s Degree in Social Work, Human Services Administration, Non-profit Management or equivalent years of relevant work experience is required
- A minimum of four (4) years of program design, curriculum development and/or professional development training, social service delivery experience is required
- Experience creating, developing, and implementing SDoH programs is preferred
- Supervisory or Leadership experience is preferred

- Intermediate proficiency level with Microsoft Office, including Outlook, Word and Excel skills
- Ability to operate smart phone, iPad, or other mobile communication devices to ensure productivity and ability to perform essential functions
- Strong collaborative skills, working with cross-functional stakeholders and external partners including state agencies, elected officials, community partners and business leaders
- Proven track record of leading teams focused on the consumer / member experience that demonstrated empathy and compassion and tracked results

CareSource requires annual proof of Influenza vaccination for designated positions during Influenza season (October 1 – March 31) as a condition of continued employment. Employees hired during Influenza season will have thirty (30) days from their hire date to complete the required vaccination and have record of immunization verified.

Working Conditions:

- Flexible hours, including possible evenings and/or weekends as needed to serve the needs of our members and may refer members to other CareSource’s resources
Compensation Range:
$61,500.00 - $98,400.00CareSource takes into consideration a combination of a candidate’s education, training, and experience as well as the position’s scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
