About The Role

We are seeking a TCM Care manager Supervisor to provide administrative, clinical, and operational oversight to Tailored Care Management (TCM) staff serving Behavioral Health members in accordance with North Carolina Medicaid Tailored Care Management requirements, state and federal regulations, and organizational policies. The Supervisor ensures the provision of high-quality, person-centered care management services designed to address members’ behavioral health, physical health, pharmacy, unmet health-related resource needs, and social determinants of health.

  • Provide direct supervision, guidance, coaching, and performance management to assigned TCM Care Managers and support staff.
  • Ensure care management services are delivered in accordance with the NC Medicaid Tailored Care Management Provider Manual, organizational policies, and contractual requirements.
  • Monitor staff caseloads, productivity, engagement expectations, outreach activities, documentation timeliness, and quality indicators.
  • Review and approve assessments, care plans, transition documentation, and other required clinical documentation for accuracy, completeness, and compliance.
  • Support staff in identifying and addressing members’ behavioral health, physical health, pharmacy, housing, transportation, food insecurity, employment, educational, and other social determinants of health needs.
  • Ensure staff provide person-centered, trauma-informed, culturally competent, and recovery-oriented care management services.
  • Participate in hiring, onboarding, orientation, and ongoing training of care management staff.
  • Conduct routine supervision sessions, team meetings, audits, and performance evaluations in accordance with agency standards.

Compensation 

This role is compensated on a salaried basis, providing consistent earnings aligned with the responsibilities and expectations of the position.

Placement within the range is based on licensure level, education, and experience

What You Can Expect 
  • Supervise and support a team of Tailored Care Management (TCM) staff to ensure quality member care.
  • Monitor caseloads, documentation, and performance to maintain compliance with NC Medicaid requirements.
  • Provide coaching, training, and guidance to help staff succeed and grow professionally.
  • Help ensure members receive person-centered support for behavioral health, physical health, and social needs such as housing, transportation, and food access.
Responsibilities 
  • Lead, supervise, and support Tailored Care Management (TCM) staff to ensure high-quality care management services.
  • Review and approve assessments, care plans, and documentation for accuracy, quality, and compliance.
  • Monitor team productivity, caseloads, member engagement, and performance expectations.
  • Recruit, train, coach, and evaluate staff while promoting person-centered, trauma-informed care.
QualificationsQualifications
  • Active North Carolina license, provisional license, certificate, registration, or permit issued by the governing board regulating a human service profession required, including but not limited to LCSW/LCSWA, LCMHC/LCMHC-A, LCAS/LCAS-A, LMFT/LMFT-A, LPA, or Registered Nurse (RN).
  • Minimum of three years of experience providing care management, case management, or care coordination to the population being served required.
  • Minimum of one year of supervisory or leadership experience preferred.
  • Knowledge of North Carolina Medicaid Tailored Care Management requirements, Behavioral Health systems, and care coordination practices required.
  • Experience working with individuals diagnosed with serious mental illness, serious emotional disturbance, substance use disorders, or complex behavioral health needs preferred.
  • Valid driver’s license, reliable transportation, and current automobile insurance required.
  • Ability to travel within assigned service areas as needed.
  • Proficiency in electronic health record systems, Microsoft Office applications, and care management platforms required
