Pay Scale: 55k (provisional)- 60k (fully Licensed)

Summary: 

This position is responsible for the oversight and supervision of the Tailored Care Management (TCM) Specialty Teams. The TCM Specialty Teams are responsible for providing comprehensive TCM services to identified populations. This position reports directly to the Program Director of TCM Specialty Teams. This position is responsible for all care management operations and compliance with all applicable policies, procedures, regulations and laws.  Position offers on-going mentoring, coaching and training to TCM Specialty Team staff.  Assures adequate staffing at all times to meet service demand and that staff meet all training requirements.  

  • Ensures strict compliance with DHHS TCM Manual, all professional standards, IPRS/NC State, Federal Medicaid and Medicare regulations, agency policies and procedures. Ensures the provider requirements for TCM are followed.
  • Provides clinical direction and supervision to all members of the TCM Specialty Teams. Provides or arranges for in-service training as appropriate, deliver clinical education programs, including best practices training to staff, also participates in agency-wide training programs/in-services. 
  • Monitors TCM performance measures. Assures all paperwork, billing tickets and documentation is completed in a timely manner according to established guidelines. Works with Program Director of TCM Specialty Teams to ensure that all regulatory standards are followed. 
  • Participates in the selection and hiring of TCM Specialty staff and other personnel actions as needed.
  • Serve as the liaison between the Program Director of TCM Specialty Teams, the TCM Specialty Teams, center TCM teams, and other agency departments.
  • All other duties as assigned by supervisor

Special Attributes:  Strong psychosocial, clinical assessment skills. Minimal supervision regarding use of time, able to prioritize work assignments.  Must possess strong clinical skills, a vast knowledge base of mental health and substance abuse disorders and vigor to continue the learning process. Ability to communicate effectively with professionals and clients/families. Ability to make sound decisions in emergency situations.

Supervisory Responsibilities:  Provides supervision to clinical care managers, extenders (based on ratios in most current DHHS TCM Manual), and administrative oversight to support teams.

Education and/or Experience: Master’s level fully or provisionally licensed LCSW, LCMHC, LPA, LMFT or RN.  Three years’ experience providing care management, case management, or care coordination to the population served.
