Role Overview
As the Supervisor of Integrated Case Management, you will oversee daily case management operations, guiding a team of professionals in delivering collaborative, patient-centered care. Your role is critical in ensuring the team effectively assesses, plans, coordinates, and evaluates care to meet the comprehensive health needs of individuals and families, promoting patient safety, quality of care, and cost-effective outcomes.

- Oversee daily case management operations, ensuring the team delivers high-quality, patient-centered care.
- Monitor and guide case management professionals in assessment, planning, facilitation, care coordination, evaluation, and advocacy.
- Participate in the development of policies, procedures, and daily workflow design.

QualificationsWhat You Will Need
Required Qualifications
- Bachelor's degree in nursing or related professional field (e.g., social work, counseling, health education) or a Master's Degree of Social Work.
- Three (3) years of clinical experience in case management, disease management, health coaching, nursing, or home health care.
- Strong communication and interpersonal skills.
- Strong analytical, problem-solving, and critical thinking skills.
- Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
Preferred Qualifications
- Certification in Case Management (CCM) from the Commission for Case Management Certification (CCMC).
Skills Required
- Bachelor's degree in nursing or a related professional field, or a Master of Social Work degree
- Three years of clinical experience in case management, disease management, health coaching, nursing, or home health care
- Strong communication and interpersonal skills
- Strong analytical, problem-solving, and critical-thinking skills
- Valid, unrestricted State of Michigan RN or LMSW license
