What We’re Looking For


Trillium Health Resources has a career opening for a STAR Coordinator to join our team! The STAR Coordinator is responsible for providing care management and transition coordination for members enrolled in the Trillium Support Transition and Reentry (STAR) Program. This program serves adults who are justice-involved and preparing for release from Department of Corrections or county detention facilities, with co-occurring Behavioral Health (BH) and/or Intellectual and Developmental Disabilities (IDD) needs. The STAR Coordinator is responsible for cross-system care coordination that addresses medical, behavioral health, social determinants of health (SDOH), and community reintegration needs. This includes linkage to disability benefits, housing resources, employment supports, community-based services, and treatment providers.


On a typical day, you might:


  • Ensure members are effectively linked to medically necessary services and community supports through comprehensive, person-centered transition planning.
  • Provide care coordination for members participating in Transitions to Community Living (TCL) and reentry initiatives.
  • Develop and implement discharge and transition plans, including supported housing, in-reach, and community outreach activities.
  • Document member progress, referrals, and engagement activities. Maintain documentation in Trillium’s Care Management systems and TCL database in accordance with policy.

Qualifications


Required:

  • Must meet Qualified Professional (QP) status as defined below with at least one (1) year experience with the forensic population (working with individuals involved in the criminal justice system).
    • Fully licensed by the NC governing board regulating Human Services professions; OR

Fully licensed RN who is licensed to practice in the state of NC by the NC Board of Nursing who also has four (4) years of full-time experience with the MH/DD/SA population served. OR

  • A graduate of a college or university with a Master’s degree in a human service field and has one year of full-time, pre or post-graduate degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has one year of full-time, pre or postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling; OR
  • A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has two years of full-time, pre or post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; OR
  • A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has four years of full-time, pre or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
  • Must have a valid driver’s license.
  • Must reside within North Carolina within 75 miles of Trillium’s Mid State Region or Southern, which includes the following counties: Anson, Guilford, Montgomery, Randolph, Richmond, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, and Scotland. 
  • Must be able to travel within catchment as required.


Preferred:

  • Degree in Human Services or Criminal Justice.
  • Education, training, and work experience that shows a history of learning and applying the skills needed to work with a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diverse backgrounds.
