About the Role 
The Support Coordinator serves as the primary advocate and resource broker for individuals and families. This role ensures access to services, entitlements, and supports while promoting Person-Centered and Family-Centered Planning principles. Responsibilities include developing and implementing Individual Plans of Service (IPOS), coordinating care, and fostering community connections.

Key Responsibilities include

  • Facilitate the development and implementation of Individual Plans of Service (IPOS) using person-centered planning principles 
  • Coordinate, connect, and monitor services and supports identified within the IPOS
  • Assist individuals and families in accessing community resources and benefits, including Medicaid, SSI, Children's Waiver, Autism Benefit, and other entitlement programs 
  • Support crisis planning and resolution 
  • Coordinate health and safety strategies, including discharge planning 
  • Participate in school transition planning and Individualized Education Program (IEP) meetings 
  • Promote community inclusion, self-advocacy, and relationship building 
  • Maintain accurate and timely documentation in compliance with agency, regulatory, and funding requirements 
  • Participate in required trainings and professional development 

About You
The ideal candidate has the following: 

  • Minimum of Bachelor's (or Master’s) degree in Human Services, Social Work, Psychology, Special Education, or a related field 
  • Strong connection to the mission of supporting individuals with intellectual and developmental disabilities and building trusting relationships with individuals, families, providers, and community partners 
  • Detail oriented and comfortable completing paperwork in a timely manner 
  • Enjoys the idea of an independent position that involves driving to the individual’s served sites and creating your own schedule 
  • Valid Michigan driver's license, proof of insurance, and an acceptable driving record

Skills Required
