The Case Management Supervisor will play a pivotal role in leading and supervising a team of dedicated case managers. Your primary responsibility is to ensure the delivery of high-quality case management services to our clients. This involves overseeing the development and implementation of case management plans, monitoring team performance, and maintaining compliance with relevant standards and regulations.

Essential Job Duties

  • Provide guidance, direction, and mentorship to a team of case managers, fostering a collaborative and supportive work environment.
  • Oversee the development and execution of comprehensive case management plans, ensuring they align with the organization's goals and client needs.
  • Implement and monitor quality assurance processes to guarantee the effectiveness and accuracy of case management services provided by the team.
  • Facilitate ongoing training and professional development opportunities for case management staff to enhance their skills and capabilities.
  • Conduct regular performance evaluations, provide constructive feedback, and address performance issues within the case management team.
  • Act as an advocate for clients, ensuring that their needs are met and that they receive appropriate and timely services.
  • Work closely with other departments and stakeholders to facilitate seamless communication and collaboration in delivering holistic care to clients.
  • Ensure accurate and timely documentation of case management activities, maintaining compliance with organizational and regulatory standards.
  • Utilize data analysis to identify trends, areas for improvement, and opportunities to enhance the efficiency of case management processes.
  • Drive continuous improvement initiatives within the case management department, contributing to the overall enhancement of client services.

KNOWLEDGE REQUIRED:

Required and preferred knowledge and experiences to succeed.

  • Education:
    • Bachelor’s degree in Social Work, Human Services, Public Health, or a related field required. Master’s degree preferred.
  • Work Experience:
    • Proven case management experience, with one year in a supervisory or leadership role required.
    • Strong knowledge of social service programs, community resources, and best practices in case management.
    • Valid Government Issued Driver’s License required.
  • Technical Experience:
    • Ability to manage multiple databases.
  • Soft Skills:
    • Exceptional leadership and interpersonal skills.
    • Excellent organizational and time-management abilities.
    • Strong analytical and problem-solving capabilities.
    • Effective communication and collaboration skills.
    • Commitment to maintaining the highest standards of client care.
  • Additional Preferred Qualifications:
    • HIV experience in a not-for-profit environment.

Salary

$60,000/annually to $70,000/annually (Depending on Experience)

The final salary will be determined based on factors such as market data, internal equity, candidate experience and qualifications. Please note that it is uncommon for an individual to be hired at or near the top of the range.
