Position Summary:

Supervised Release offers an alternative to jail, providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and non-violent felony offenses. Program participants are monitored to ensure their appearance at court dates, and they receive referrals to community-based programming. Under the Supervised Release program (SRP), the Supervising Case Manager (SCM) will be responsible for, supervising Case Managers and managing a small caseload, conducting intake and assessments for SRP clients and developing individualized service plans to provide case management services; providing counseling, appointment reminders, and referrals (including setting up appointments for voluntary services and following up on service uptake); ensuring individuals return to court; and, entering real time data on all client activities.

Communication:

The SCM must be able to communicate effectively, with internal and external program staff, and participants. The SCM must be able to maintain communication with these entities to ensure their clients are being assisted in a timely and effective manner.


Time Management:

The SCM will receive a large number of referrals, sometimes at one time, and must provide up to date and accurate information to the Court Advocates and SCM Supervisors on their client’s progress and participation. The SCM must maintain communication with these internal staff as well as any parties overseeing voluntary services that their clients are enrolled in. Additionally, the SCM will need to conduct client screenings, and field calls from active clients and internal and external program staff. Prioritizing the above is imperative in being able to manage the work.


Problem Solving:

The SCM has the unique responsibility of being the main point of contact for the client, and sometimes the most trusted; they are at the forefront of managing a client’s needs and must be equipped with the ability to problem solve when issues arise.


Essential Duties and Responsibilities:

  • Conduct intake interviews and assessments for SRP clients;
  • Perform general Case Management duties, including one-on-one check ins with clients (frequency dependent on supervision level), facilitate voluntary referrals to internal and community-based resources, developing client schedules, and client centered supervision plans to help address such issues as substance abuse, mental health, trauma, domestic violence, and housing;
  • Assist clients in identifying and prioritizing needs;
  • Provide voluntary referrals to ensure participants’ concrete needs are addressed;
  • Provide crisis intervention when needed in collaboration with supervisor;
  • Collaborate with Court Advocates and Community Engagement Specialists;
  • Ensure accurate and timely data entry of all client information into the agency’s database;
  • Monitor participants attendance and compliance with supervision requirements;
  • Advocate for individual client services;
  • Participate in case conferences for optimum collaboration among internal programs and external agencies;
  • Develop and maintain relationships with community-based organizations;
  • Attend and participate in all necessary SRP meetings, conferences, and trainings;
  • Engage in his/her own ongoing professional development; and
  • Additional responsibilities as required.

Qualifications

Qualifications:

· High school diploma or GED/HSE with 3-4 years of relevant experience required; Bachelor’s degree in Social Work or another related field desired; LMSW desired.

· Experience and commitment to working with criminal justice involved participants;

· Ability to work with people from diverse backgrounds in a culturally-competent manner;

· Strong Microsoft Office and Excel, and database entry skills;

· Strong assessment and engagement skills;

· Strong written and verbal communication skills;

· Bilingual in Spanish a plus;

· Strong organizational and leadership skills.
