We need a full-time STNA Social Services Designee to join our team at The Advanced Therapy Center at Chesterwood Village in West Chester, OH.
You’ll assisted our Licensed social Worker by ensuring that the medically-related emotional and social needs of our residents are individually met and maintained.
$16.50 to $20 Per Hour
You’ll Be Responsible For:

Assisting with gathering information from residents and family members
-
Assisting with the completion of the Integrated Personal Services Assessment and MDS
-
Assisting with the discharge planning process by arranging community resources, alternative placement, etc.

Attending Care Conferences as directed by the LSW
-
Assisting LSW with any other case management tasks deemed necessary to the Department.

Fulfilling basic documentation requirements for social services.

Assisting (after training) in the BIMS and the PHQ-9
You’ll Need To Have:

An STNA certification or eligibility and experience in long term care.

Excellent communication skills
-
A High School Diploma and/or GED
-
Healthcare industry, social work, or other related experience

The ability to multi-task in a fast-paced, high volume environment

Exceptional organization skills with strong attention to detail

A personable and professional demeanor
-
Computer proficiency in Excel, Word, Power Point, and Outlook
