Responsibilities:
- Perform initial telephonic outreach to members in support of the care management team
- Supports the Nurse Care Manager in the completion of HRA and other SNP activities
- Return all calls received for Behavioral Health Care Manager members daily
- Attempted outreach to Behavioral Health members weekly (TOC, HRA, MOC F/U calls returning voicemails, call post-MD and HEDIS appointments)
- Outreach to all members post-ER visit
- Outreach to all members post d/c from IP admissions (ACH, SNF, LTACH)
- Assist with scheduling follow-up appointments and set up transportation if needed. Complete follow-up calls to ensure member attended appointment(s)
- Assist Member in Enrolling in Qualified Government Subsidy Programs
- Manages calls for the team utilizing standard protocols
- Communicates with members, family, or caregivers when appropriate, as directed by care management team
- Researches all administrative issues before directing to the care management team
- Prioritizes and organizes own work to meet turnaround time and deadlines
- Processes work as assigned within established timeframes and productivity goals using department resources and procedures with minimal oversight
- Follows record management and documentation protocols
- Supports peers on the team with other duties to help maintain turnaround times
- Successful completion of call and documentation audits
Minimum Qualifications:
- Bachelor’s or Master’s Degree in Social Work
- 5+ years of clinical social work experience
- Knowledge of specific software programs used
- Excellent written and verbal communication with an emphasis on clarity and compassion
- Ability to make psychosocial assessments and develop and implement care plans
Preferred Qualifications:
- Experienced in caring for patients with Serious Mental Illness
- Experienced in caring for patients with co-occurring substance use disorders
