Position Summary

We are seeking a compassionate and organized Social Services Director to oversee and provide medically related social services that support the emotional, social, and psychosocial well-being of our residents. This role collaborates closely with residents, families, and the interdisciplinary team to ensure quality care and successful outcomes.

Key Responsibilities
  • Assess residents’ social, emotional, and family needs and develop appropriate care plans
  • Provide emotional support, counseling, and guidance to residents and families
  • Assist residents in achieving the highest level of independence and well-being
  • Coordinate with community agencies and resources to meet resident needs
  • Support discharge planning, including referrals, equipment, and services
  • Schedule and coordinate outside services (e.g., podiatry, dental, psychiatric care)
  • Assist residents and families with care decisions and transitions
  • Maintain accurate documentation of social services interventions
  • Respond to concerns regarding resident belongings and personal needs
  • Collaborate with interdisciplinary team members to ensure quality care
  • Provide staff training and in-services as needed
Qualifications
  • Bachelor’s degree in Social Work or Human Services (required)
  • MSW preferred
  • Minimum 2 years of supervised social work experience in a healthcare setting
  • Experience in skilled nursing or long-term care preferred
  • Strong communication, organization, and documentation skills
  • Ability to handle sensitive information with confidentiality and professionalism

Skills Required
