The CDO Support Broker/Case Manager is responsible for coordinating services for individuals that have selected Consumer Directed Options, a choice under the Supports for Community Living, Michelle P. and Acquired Brain Injury waivers. The CDO Support Broker is a positive team player that promotes a culture of inclusion, and participation.

Required Education and Experience

  • Bachelor’s degree in a human services field.
  • Valid driver’s license, reliable operating transportation, and proof of current auto insurance

Preferred Education and Experience

  • Experience working with IDD population and Medicaid Waiver programs
  • SCL (Supports for Community Living) Intellectual Disability Professional  

Essential Functions

       Case Management

  • Complete assessments to determine eligibility for ongoing services
  • Maintain client plan of care that is reflective of needs as identified by interdisciplinary team
  • Develop crisis prevention plan and connect participants with additional community-based services to help prevent institutionalization
  • Process employee paperwork and coordinate training prior to service provision
  • Complete required documentation and training
  • Participate in and promote performance improvement activities

Cost Containment and Productivity

  • Work collaboratively with other region-wide programs to determine services offered in the Region
  • Ensure that staff and other resources within the Region are utilized in a manner that optimizes the service delivery system

Corporate Involvement and Responsibilities

  • Actively participate in corporate-wide initiatives
  • Support and work within the mission and strategic plan of the organization
  • Ensure compliance with relevant laws, regulations and standards. e.g., state contracts, Joint Commission, etc.
