Supports the Social Services Department in the admission and discharge process. Serves as the back up to the Director of Social Services. Works with social services to ensure the psychosocial needs of the residents and patients are met.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.• Assists in the completion of care plans and psychosocial assessments.• Coordinates care plan meetings with families.• Coordinates, notifies and documents room changes and roommate notifications when necessary.• Conducts tours of facility with potential residents / patients and their family members.• Ensures specialized equipment is ordered as indicated (reviews contract content for need to obtain prior approval from authorized payer).• Completes records and documentation in accordance with company policy and state and federal guidelines.• Helps resolve concerns of Resident and/or Family and participates in the Guardian Angel Program.• Provides patient comfort by utilizing resources and materials; transporting patients; answering patients’ call lights and requests; reporting observations of the patient to nursing supervisor.• Primary Back-Up for the Director of Social Services when out on PTO, FMLA or the Director of Admissions position is open.• Protects organization’s value by keeping patient information confidential. • Services and protects the facility by adhering to professional standards, company policies and procedures, federal, state, and local requirements, and standards.
