Alhambra Post Acute is seeking a compassionate, organized, and resident-focused Social Services Assistant to join our team in Martinez, California.

In this role, you'll be a vital advocate for our residents and their families, helping address social, emotional, and psychosocial needs while supporting quality of life, care planning, and successful transitions throughout the healthcare journey.

What You'll Do
  • Provide medically related social services to help residents achieve and maintain their highest practicable physical, mental, and psychosocial well-being
  • Assess residents' social, emotional, and family needs and contribute to individualized care plans
  • Offer emotional support and counseling to residents and families coping with illness, disability, grief, and life transitions
  • Assist residents and families with care planning and healthcare-related decisions
  • Coordinate discharge planning with the interdisciplinary team, including referrals for community resources, services, and equipment
  • Schedule outpatient appointments and ancillary services including dental, optometry, podiatry, and psychiatric visits
  • Connect residents and families with community programs and support services
  • Assist with personal shopping requests and follow up on lost or misplaced belongings
  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements
  • Collaborate with nursing, therapy, administration, and community agencies to support resident success
What We're Looking ForRequired
  • Licensed Social Worker Certification
  • Strong communication and interpersonal skills
  • Excellent organization, time management, and problem-solving abilities
  • Ability to build positive relationships with residents, families, staff, and community partners
Preferred
  • Skilled nursing or healthcare experience
  • Experience with PointClickCare (PCC)
